4-1A - food separated and protected
Observed several food separation violations within the walk in cooler. Ready to eat produce was stored directly underneath raw ground corned beef hash on the rack. Observed raw whole cut steaks on speed rack stored below raw ground burger, pork and corned beef hash. CA: PIC moved all food items into proper separation storage
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(a) - handwashing cleanser, availability (pf)
Observed no operational hand soap dispenser, at the handwashing sink behind the bar by the main grill. CA: PIC located new hand soap and placed at the sink. RCA: Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
12A - contamination prevented during food preparation, storage, display

observed raw fruits and vegetables to be stored on the ground in the walk in cooler. CA: PIC moved items onto the very top of the storage rack.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
observed flooring throughout the kitchen to no longer be smooth, non-absorbent, and easily cleanable. Observed holes in the walls in the kitchen next to the ware washing machine. RCA: All physical facilities shall be maintained in good repair.
