4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed knife stored as clean but visibly dirty. Knife strip was also visibly dirty with rust accumulations. COS by washing knife. Ridged knife strips must be changed with an all stainless steel strip for easy cleaning.
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(b) - working containers, common name (pf)
Observed several chemicals stored in unlabeled containers near mop sink. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. COS by labeling.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed several small, flying insects in warewash/mop area. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and
-
Eliminating harborage conditions.
Must increase pest control frequency and clean up area.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Physical facility is in need of deep cleaning. Must clean the exterior, beneath, and behind all equipment (e.g., fryers, grills). Must also thoroughly clean mop sink area. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
*This has been a recurring issue over at least 3 inspections. Health Authority will follow-up on cleaning after 7 days. Failure to correct deficiency may result in facility being asked to close until proper cleaning can be conducted .
