Observed employees preparing food without wearing hair restraints. Must tie hair back and wear hat or hairnet if hair is 0.5 inches or longer. COS by putting on visors.

Observed debris accumulation on drawers storing clean utensils (across from 3 compartment sink). Also observed debris accumulation on floors of walk in cooler and walk in freezer. Nonfood contact surfaces shall be cleaned at a rate that precludes soil accumulation. Must clean more frequently.

NOTE: All loose hair must be restrained within hair restraint.

Observed ladles stored upright and exposed to contamination. One ladle was in direct contact with a music speaker. Knives, forks, and spoons that are not pre-wrapped shall be presented so that only the handles are touched by employees and by consumers if consumer self-service is provided. Must rewash and store with handle upright.
