Observed signed copies of Employee Health Policy missing for several employees. Must have signed copies of EHP for all current employees.

Observed significant debris accumulation on floors of main kitchen area, especially behind pizza oven and reach in coolers. Also observed significant grease build up on hood vents above grill. All physical facilities must be cleaned as often as necessary to keep them clean. Must clean more frequently.

Observed time/temperature control for safety (TCS) food in front prep top unit and walk-in cooler being cold held at temperatures greater than 41°F. Ambient temperature of prep top unit was about 54°F and ambient temperature in walk-in was 41°F. [See temperature observations]. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F or below when being cold held. COS by discarding. DO NOT STORE TCS FOODS IN COOLERS UNTIL PROPER COLD HOLDING TEMPERATURES CAN BE MAINTAINED.
*Back pizza prep top was brought out to front until other unit can be fixed.

Observed bowl of mushrooms stored directly under condensation/leak in walk-in. There was visibly water from leak on exterior of foil covering mushrooms. Food must be stored in a manner where it is not exposed to splash. COS by moving. Do not store food in area until fixed.

Observed mold build-up along interior portions of bulk ice machine. Ice machine must be emptied, cleaned, and sanitized. Ice machine must be emptied, cleaned, and sanitized within 24 hours. Must increase cleaning frequency to a rate that precludes mold accumulations. Maintenance tech stated he will clean on 12/11/23.

Observed drink pitchers stored on dirty shelving. Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS by removing pitchers and sending to dish pit for wash/sanitizing. Shelves must be cleaned and sanitized.

Observed chemicals and drinks stored directly on floor. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food:
(i) In a clean, dry location;
(ii) Where it is not exposed to splash, dust, or other contamination; and
(iii) At least 6 inches (15 cm) above the floor.
Corrective action: items must be moved off floor.

Observed grease build-up on hood vents and mold on fans in the walk-in cooler. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Must correct within 48 hours.

Physical facilities are in severe disrepair as a result of neglect. Areas that must be addressed:
PERSON IN CHARGE MUST ENSURE CORRECTIONS ARE MADE WITHIN 60 DAYS AND MAINTAIN FACILITY IN A STATE OF GOOD REPAIR MOVING FORWARD. HEALTH AUTHORITY WILL FOLLOW-UP TO ENSURE CORRECTIONS HAVE BEEN MADE.
