Observed exposed concrete floors in main kitchen area near 3 compartment sink and back door. Also observed gaps of exposed concrete in between panels of smooth material in main kitchen area, with significant water accumulation. All physical facilities must be maintained in a state of good repair. Must replace flooring. PIC states they are in the process of remodeling the floors.

Observed significant food debris build up under shelves next to grill in main kitchen area. Also observed debris build up on floor next to ice machine in front food service area. Physical facilities must be cleaned as often as necessary to keep them clean. Must sweep.

Observed employee drink stored directly on prep counter where food was actively being prepared. Employee personal items and drinks must be stored in a separate, designated area away from food prep. COS by throwing away drink.

Observed several time/temperature control for safety (TCS) foods cold held at temperatures above 41°F in reach in cooler (RIC) directly under prep top unit. The ambient air temperature (AAT) is 41°F. Also observed TCS items cold held in prep top unit at temperatures above 41°F. Health authority suggested lowering AAT to 36 or 37°F. PIC said she will get maintenance to look at cooler. COS by moving all TCS items to walk in cooler.

Observed a hand washing sink obstructed with a nail brush. All hand washing sinks shall be accessible at all times. COS by moving nail brush.

Observed handwashing sink missing paper towels. All handwashing sinks must be stocked with paper towels at all times. COS by restocking paper towels.
