Employees did not wash hands upon reentering kitchen from outside. CA: Employee to wash hands after contamination from handling raw meat, handling trash, reentering kitchen from rest room or from outside. COS- Employees stopped duties, and washed hands at hand sink. Repeat.
4-2B - food-contact surfaces: cleaned & sanitized

Employee rinsed knife and did not clean, rinse and sanitize utensil as required. CA: Utensils to be properly washed, rinsed and sanitized after use. COS- PIC washed, rinsed and sanitized knife.
