Dispenser tube stored in kitchen hand wash sink. CA: Keep hand wash sink accessible at all times and used for no purpose other than washing hands. COS- Employee removed tube from kitchen hand sink.

Employee not wearing effective hair restraint during food preparation. Repeat.
