Observed ice baths set up incorrectly. Food sitting on top of ice. Person in charge (PIC) re-made ice baths during inspection with a 50/50 mixture of ice and water. Containers submerged so that ice was at food level. REPEAT C.O.S.

Observed bowl without handle used as scoop for brussels sprouts in grill make table. Scoop removed during inspection. C.O.S.
