Observed raw lamb stored over fish and ready to eat food, and raw chicken stored over ready to eat foods in reach in cooler. COS: Had PIC rearrange items based on minimum internal cooking temperature.
18 - insects, rodents, and animals not present

Observed large hole in back door. Hole shall be repaired so to prevent entry of pests
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed Nonfood-contact surfaces of crock pots and toaster oven with debris and build-up on outside surfaces. Advised PIC non-food contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. COS: Had PIC place items at 3-compartment sink for cleaning.
17C - physical facilities installed, maintained, and clean

Observed several large holes in walls in multiple areas. floors along base of walls and under equipment in kitchen have build-up present. Advised to clean.
