Observed the paper towel dispenser inside of the restroom in disrepair - was not able to pull paper towels from the dispenser. COS: Employee repaired the unit.

Buildup of food debris on the surface of the dishes/pans (food contact surface). Explained to PIC that food debris should be removed from surfaces and food contact surfaces sanitized.

Observed PIC wash hands in cold water then proceeded to handle pizza. Advised PIC on proper cleaning procedures. COS: PIC rewashed hands with warm water at the designated handwash sink.

Observed food employee come into the building and proceeded to handle RTE boneless chicken without washing hands. Advised PIC that all employees shall wash hands prior to handling food once entering the building. COS: Food employee washed hands at the designated handwash sink.

PIC could not demonstrate in a manner that food employees are/were informed of their responsibility to report symptoms/illnesses related to foodborne diseases. Advised PIC that an employee health policy and agreement forms shall be singed and made available upon request of the Health Authority. CA: Advised PIC to have agreement forms signed.

Observed food contact surfaces of metal pizza pans with excessive carbon buildup. Advised PIC that all food contact surfaces shall be cleaned to sight and touch. CA: Advised PIC to wash, rinse, and sanitize pizza pans after every use and to discard pizza pans that cannot be cleaned from excessive carbon buildup.

Observed multiple food employees prepping and handling food with witch watches on wrists. Advised PIC that when handling food and clean utensils, employees shall not wear jewelry on hands or wrists with an exception of a simple wedding band. COS: Food employees removed watches from wrists.

Observed food contact surfaces of multiple metal pizza pans and plastic containers stored as clean with black/brown residue and food debris. Advised PIC that all food contact surfaces shall be clean to sight and touch. Also advised PIC that metal pizza pans shall be washed, rinsed, and sanitized after every use and to discard pizza pans that cannot be cleaned to sight and touch.
