Observed the following rooms with stains/debris on the pull out mattress: 213, 222, 314. Observed stains on the pillows in the following rooms: 402, 314, and 213. CA: Beds, mattresses, mattress pads, pillow, and pillow coverings shall be clean and free from vermin

Observed the following rooms with soap scum on the shower heads: 213, 222, 314, and 402. CA: Toilet rooms and fixtures must be smooth and non absorbent and shall be kept clean and in good repair

Observed ice machines on 3rd floor hallway and 4th floor hallway with pink build-up. CA: Ice machines shall be maintained and shall be kept clean

Observed debris and rusting in microwave for rooms 416, 303, 212; stains on curtains for rooms: 322, 212. Advised PIC all furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed residue/brown staining on showerheads for rooms: 416, 303, 322, 206, 414, 402, 212; crack in shower floor for room 303; stain on towel room 416. Advised PIC Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed tears on ceiling of Bathroom for rooms: 112 and 324. Advised PIC toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed ice machine on 2nd floor, 3rd Floor, and 4th Floor interior food contact surface in need of cleaning with debris, stains, and residue. Advised PIC equipment shall be properly cleaned and sanitized according to the manufacturer’s recommendations

Observed housekeeping cart on 3rd floor, personal bag on clean linens. Observed housekeeping cart on 4th floor, face and body wash stored directly next to clean linens. Advised PIC housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected. COS: Had employee relocate items away from clean linens

Observed holes in curtains for rooms: 211, 213, 313. Observed stains on lampshade room: 211, 213. Observed mattress covers with stains in rooms: 104, 211, 213, 313. Advised PIC beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Oberved ice machine on 1st floor with debris on interior food contact surface. Also, Observed multiple ice machines in disrepair (floors: 3rd & 4th), without any out of order signs placed on unit. Advised PIC that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed stains on mattress in rooms 200, 218, 307, 326, 418, 413, 411. Observed stains on pillow case in room 413. Advised PIC all bedding shall be maintained at all times

Observed single use coffee cups and lids in 144 & 106 unwrapped. Advised PIC all single use items shall be protected from contamination at all times

Observed pink mold-like accumulation on the surfaces of the ice machine on floor 1st, 2nd, 3rd, & 4th. Advised PIC to clean at a frequency to minimize accumulation

Observed chemical bottles on laundry carts and in the laundry room unlabeled. Advised PIC chemical bottles shall be labeled at all times

Observed food debris in microwave in rooms 407, 403, 402, and 303; Observed staining on lamp shade in room 402. Advised PIC that all bedroom furniture and room accessories shall be kept clean and in good condition at all times

Observed mattress, bed linens, and mattress cover with severe staining in rooms 407, 403, 420, 320, 303. Also observed staining on pull-out mattress in rooms 106, 420, and 203. Advised PIC that all mattresses, bed linens, and mattress covers shall be kept clean and in good condition at all times,

Observed staining on bathroom ceiling in rooms 407, 403, 402, 203; observed stains on bathroom towels in room 320; observed debris on bathroom vent in room 402. Advised PIC that all bathroom facilities and its fixtures shall be kkept clean and in good repair at all times

Upon arrival, observed coffee stirrers and lids not individually wrapped. Advised PIC that single-service coffee stirrers a d lids shall be protected against contamination. Advised PIC to obtain individually wrapped lids/stirrers or to obtain a dispenser

Observed 4th, 3rd, 2nd, and 1st floor ice machines with brown/pink mold-like substance on white panel. Advised PIC that all ice machines shall be maintained so that they are kept clean at all times
