Observed several instances where employees are not washing hands when required to such as: Employee returned from outside and began touching utensils in main kitchen without washing hands first; Employee cleaning counter tops then began preparing orders without washing hands first (main kitchen); Employee scratching head while cooking burgers without washing hands (grill prep); employee taking orders and payments then returned to main kitchen and began touching utensils (main kitchen).

C/A: Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles.

COS: Manager educated and instructed food employees to wash hands.

Observed box of orange lava burst (beverage) directly on floor in dry storage area. C/A: food shall be protected from contamination by storing in a clean, dry location where it is not exposed to splash, dust, or other contamination; and at least 6 inches (15 cm) above the floor. COS: Removed from floor
