Observed live roach and roach droppings in room 271. Observed flies in room 295. Observed fly consuming a sticky substance on a table in room 271. Observed rat droppings in room 195; PIC stated this room was not ready for occupancy.
CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation
Regulation: 511-6-2-.14(5) - condition of furnishings
In room 295, observed dirty towel, used razor, and glasses underneath furniture. In room 283, observed a broken chair, misaligned bed frame that caused metal brackets to protrude into the walking path for bed access, and trash under the bed. C/A: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed stains and hairs on mattress covers, mattresses, and pillows in all rooms.
CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

In room 295, observed toilet with unflushed debris inside. Flies in bathroom. Shower head missing with foil wrapped around pipe. In room 271, bathroom vent was heavily caked with dust build up.
C/A: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means
Regulation: 511-6-2-.13(3) - insect and rodent control-openings
Observed a broken window frame in room 271 that was not aligned properly. No screen on window.
C/A: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation
Regulation: 511-6-2-.14(1) - floors, walls, and ceilings
Observed dirty carpets, stained walls, and sticky spots on bathroom floors in each room. Trash accumulated under most furnishings. Missing electrical outlet covers in rooms 283 and 263.
C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean
