Observed loose and removable shower knob in room 416. C/A: Shower knob must be secured to the shower/tub and in good condition

Observed, on 4th floor housekeeping cart, 2 spray bottles, 1 with a green liquid and 1 with a clear/hazy liquid, with no label or name of chemical on the spray bottle. C/A: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. COS: Employee stated they are a bleach solution and glass cleaner

Observed staining on mattress pads, bedbug covers, and pillows in rooms 106, 201, 322, 405, and 410. Staining on bedbug cover of pullout couch in room 410. CA: Bedding linens, bedbug covers, and mattresses should be free of stains and in good condition

Observed loose and removable shower knob in room 405 CA: Shower knob must be secured to the shower/tub and in good condition

Observed paint peeling on ceiling of shower in room 201.

CA: Floors, walls, and ceilings must be clean and in good condition

Missing anti slip material in room 405.

CA: Anti slip surfaces should be provided and in good condition

Observed water exceeding 120F temperature in rooms 418 (127F), 316 (123F), 235 (130F), 205 (126F), 105 (122F) CA 511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. Facility has 30 days to correct the violation

Facility not able to provide a gas and fire inspection report due to the fact that they could not pass the inspection. CA 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency NEED TO PROVIDE THE ANNUAL GAS AND FIRE IN 30 DAYS

Observed yellow stains on the mattresses under the protective cover in rooms 418, 316, 311, 235, 205 and 105. CA 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed the water stopper in the tubs and the sinks in every room I inspected missing or loose. CA Water Stopper needs to be secured to the tub or sink in order to stop the water from draining. Facility must repair in 30 days

Observed an accumulation of hair strands on sheets and mattress pads in rooms 102, 206, 333, and 424. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed chemical spray bottles on 2 housekeeping carts (on level 1 and 3) without labels. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
