Observed multiple items cold holding above 41F in the stovetop cooler, ice bath, and flat-top cooler in main kitchen (see asterisk).

CA: When cold holding, TCS items shall be maintained at 41F or below.

COS: Upon discussion with PIC, EHS learned that the units are turned off at night and turned back on in the morning. Recommended that staff check the temperatures of the units to ensure they are 41F or below before the TCS items are placed in the units. Ice bath items did not have sufficient ice to maintain temperature. All items had been placed on the line about 1 hour ago - items were allowed to cool and placed in the walk-in cooler for rapid cooling. Some items were discarded by PIC (see asterisk)
