- Observed raw pork chops stored above ready to eat ajus sauce and margarine in WIC in main kitchen.
- Observed raw shell eggs stored above ready to eat deli meats in lower compartment of large prep top cooler in main kitchen.
- Observed raw shell eggs stored above several ready to each sauces in RIC in dry storage area of main kitchen near WIF.
C/A: All ready to eat foods must be stored above raw foods to prevent cross contamination.
COS: Food rearranged properly.
4-2B - food-contact surfaces: cleaned & sanitized

- Observed drink gun line stored directly in contact with ice for beverages at bar along front food service area.
- Observed black and pink accumulations inside drink guns at bar and in main kitchen.
- Observed can opener blade with thick black accumulation stored for use at prep table in main kitchen.
C/A: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
COS: All equipment cleaned and sanitized; ice replenished with new.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used

Observed large amounts of accumulated ice from piping of condensor and along floor in WIF in main kitchen.
C/A: 1. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule.
- Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
*Per PIC; technician is contracted to repair unit on tomorrow, 4/8/2026.
15C - nonfood-contact surfaces clean

- Observed walls of WIC in main kitchen heavily soiled with black, mold-like accumulations and residue.
- Observed standing water along base of 2-door keg cooler in bar along front food service area.
- Observed shelving in WIC in main kitchen soiled with accumulated debris and dark matter.
C/A: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
2nd consecutive violation
17C - physical facilities installed, maintained, and clean

- Observed discolored, damaged, swollen ceiling tiles with accumulated dust throughout main kitchen.
- Observed hole in wall near WIF in dry storage area in main kitchen.
- Observed damaged and missing baseboards along base of wall near entrance door to main kitchen from dry storage area and in dry storage area near exit door.
C/A: Repair all areas; All physical facilities shall be maintained in good repair.

Observed personal food (subway sandwich) stored directly on top of food service food in lower compartment of large prep top cooler in main kitchen.
C/A: Areas designated for employees to eat, drink, store food, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.
COS: Food discarded.
