Observed stains, hairs, and debris on pillows, bedding, and mattresses of all rooms observed.

C/A: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed bathroom vents not working in all rooms observed. Motors were making noise but there was no pull of air.

C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated

Observed missing window screens in rooms 112, 113, and 119.

C/A: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed floors, walls, and ceilings in all rooms with stains, dust, and debris accumulated. Surfaces of furniture, refrigerators, and faucets were stained and sticky to the touch.

C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed the following rooms with stains and/or holes in the bedding of the following rooms: 103, 107 and 114.

CA: CA: Replace all items; Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase

rooms104, 108, 107, 112, 118 observed with holes and stains in bedding.

CA: In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

Observed dust accumulated in a/c vents and filters in rooms 104, 110, and 120.

C/A: Clean all areas; All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

1. Observed heavily stained mattresses along beds in rooms 104, 110, and 120.
2. Observed heavily damaged and stained bedding along beds in rooms 104, 110, and 120.
3. Observed stained, soiled pillows in rooms 110 and 120.

C/A: Replace all items; Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

1. Observed walls peeling in disrepair in restroom of room 104.
2. Observed splatter and debris along walls in room 120.
3. Observed damaged areas of walls in rooms 104.

C/A: Clean/repair all areas; floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored

Observed black residue and accumulations along interior nozzle of ice machine.

C/A: Clean/sanitize ice machine; If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice

Observed tub in room 110 without anti-slip tubs slip strips, appliqués, or slip-proof mats.

C/A: Install/ provide anti-slip tubs slip strips, appliqués, or slip-proof mats in each bathing facility and shall be kept clean and in good repair
