3 - hot/cold running water under pressure in all required locations of tourist accommodation
Regulation: 511-6-2-.07(2) - hot water supply
Observed rooms 214 and 372 without hot water at the bath tubs. C/A: Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F
6 - premises free of pests and rodents; pesticide applied in accordance with laws, rules, and label
Regulation: 511-6-2-.13(1) - insect and rodent control
Observed the following: -Roach infested mattress in rooms 166 -Dead insects, live spider and flying insects throughout room 166 -Dead bugs inside AC unit of 585 -Live roach in kitchen area of 214 C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed the following: -Room 362: Stained mattress, no mattress covering, dirty pillow -Room 426: Stained mattress, no mattress covering C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly
17 - proper ventilation; no sign of mold, mildew, odors or moisture
Regulation: 511-6-2-.08(5) - toilet and bathing ventilation
Ventilation in bathrooms were not working in all rooms observed. Mold like accumulations found on ceilings of bathroom/vanity of rooms: 166 and 593. C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

Observed the following: -Room 362: Stained walls, peeling ceiling -Room 426: Broken blinds -Room 585: Broken blinds -Room 593: Scratched floor, loose coving in bathroom -Room 214: Cracks on groves on wall C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean
33 - housekeeping carts and storage areas maintained; clean/sanitized items protected from contamination

Observed unlabeled spray bottles on house keeping cart. C/A: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
