Observed rooms 214 and 372 without hot water at the bath tubs. C/A: Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

Observed the following: -Roach infested mattress in rooms 166 -Dead insects, live spider and flying insects throughout room 166 -Dead bugs inside AC unit of 585 -Live roach in kitchen area of 214 C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

Observed the following: -Room 362: Stained mattress, no mattress covering, dirty pillow -Room 426: Stained mattress, no mattress covering C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Ventilation in bathrooms were not working in all rooms observed. Mold like accumulations found on ceilings of bathroom/vanity of rooms: 166 and 593. C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture

Observed the following: -Room 362: Stained walls, peeling ceiling -Room 426: Broken blinds -Room 585: Broken blinds -Room 593: Scratched floor, loose coving in bathroom -Room 214: Cracks on groves on wall C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed unlabeled spray bottles on house keeping cart. C/A: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents

Observed roach infestations in rooms 181, 195, 593, and 164. C/A: Premises shall be free of pests

Observed dirt/debris on sheets and holes/tears in mattress of room 593. C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Observed room 181 with unclean tub; 593 with unclean tub. C/A: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed room 593 with heavily soiled blinds. C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed severe roach infestations in rooms: 116, 166, 181, and 195. Observed dozens of drain flies through out room 166. C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program

Observed bed frames, kitchen cabinets, counters, dressers throughout rooms 116, 164, 166, 181, and 195 are soiled and have accumulations of roach droppings. C/A: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed the following: Stained and dirty cover in rooms 116; No mattress covers in rooms 166 and 195.; Hole in linen in room 164; Stained and dirty mattress in rooms 116, 164, 166, 181, and 195; Several small spiders crawling on linens in room 181. C/A: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

Observed no toilet paper available in rooms 116 and 195. C/A: Toilet tissue shall be provided in a dispenser at each toilet at all times

Observed openings to the outside around front door frame in room 195 and 166. C/A: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks. Screening materials shall not be less than sixteen mesh to the inch

Observed stained and dirty walls, floors and doors throughout rooms 116, 164, 166, 181, and 195. Observed peeling paint and materials on walls and ceilings of rooms 116, 164, 166, 181, and 195. C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored

Facility is unable to provide gas fired equipment inspection report. C/A: Keep gas fired equipment inspection report on site at all times. Correct in 24 hrs

Observed soaps/shampoos exposed to cleaning brushes on house keeping carts inside laundry room. C/A: Protect clean items on house keeping carts from contamination. Correct in 72 hrs

Observed water leakage under bathroom sink in room 210; Observed water seeping through floor in room 210; Observed leak under kitchen sink in room 589. C/A: All plumbing in tourist accommodations shall comply with State and local laws, ordinances or regulations. In the absence of State and local laws, ordinances or regulations, the provisions of the current “International Plumbing Code with Georgia Amendments or future subsequent versions adopted thereafter shall prevail. Correct within 72 hours

Observed kitchen sink and bathroom sink exceeding 120F in room 210. C/A: Hot water in all guest rooms shall not exceed 120F. Correct within 30 days

Observed live roaches in room 118. Observed dead bed bug in room 118. Observed roach waste along walls in kitchen area in room 589. C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator. Correct within 30 days

Observed blinds with dark accumulations in room 118; Observed stains and splatters inside and outside fridge of room 118; Observed AC unit in disrepair in room 210; Observed trash and hair inside cabinet of room 275. Observed facility does not have current gas fired inspection report on file. C/A: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available

Observed water damage in cabinet underneath vanity in room 214 (occupied). Observed water damage in cabinet under kitchen counter in room 308.
