Observed ice bin located close to the bar area having heavy build-up of mold inside. Observed soda nozzle off kitchen area with mild like substance build-up C/A n equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment:

(I) At a frequency specified by the manufacturer; or

(II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. COS: Ice bin was shut down and the cleaning company was contacted. Soda bin need to be cleaned in 3 days

Observed the sani buckets with sanitizer having a concentration below 200ppm (QAT) C/A Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and

(ii) Laundered daily. COS: Facility called the company and they were going to come today to calibrate the automatic sanitizer dispenser

Observed pico de gallo, cheese, lettuce, and pickles stored uncovered in the walk-in cooler in the main kitchen. C/A: Food shall be stored covered in order to prevent the possibility of contamination. COS: Items covered.

Observed door seal on small reach in cooler was taped and in disrepair. CA:Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. PIC to repair or replace cooler by December 22. Provide documentation to Health department of repair or replacement.
