C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

NO ANTI-SLIP SURFACE OBSERVED IN FLOOR OF TUBS IN BATHROOMS

C/A: Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Observed bathroom sink temperature in "Sunny palm" (130F) and "Bella Palm" (136F) rooms exceeding 120F.

C/A: Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed; hot water in all guest rooms shall not exceed 120F.

POC: Facility must adjust water heater so that the temperature is not exceeding 120F.

EHS will return in 10 calendar days to check on the status of the hot water

PIC unable to provide annual gas fired inspection report.

C/A: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

POC: schedule a gas-fired inspection with a qualified licensed contractor or local fire authority.

Observed bathroom sink temperature above 120F in "Sunny Palm" room and "Royal Palm" room.

CA: In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F
