Observed expired food manager certification posted along bar for Tina White (Exp: 2/2026).
C/A: A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment.
Post current food manager certification and email copy to EHS within 10-days.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
- Observed soiled shelves lined with aluminum foil along several prep tables in main kitchen.
- Observed large amounts of accumulated ice and food debris along interior walls and base of RIF in main kitchen.
C/A: Clean all equipment; nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.If aluminum foil is used to line shelves, foil must be removed every day and equipment must be cleaned and sanitized.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed missing ceiling tiles (3) along corners of ceiling in main kitchen.
C/A: Replace ceiling tiles; all physical facilities shall be maintained in good repair.
