Observed raw beef, raw chicken, raw pork chops in original containers stored above ready-to-eat/pre-cooked food items (ham patties and chicken breasts) in reach in freezer near dishwasher. CA: Food shall be protected from cross contamination except as specified in paragraph (i)(IV) of this subsection, separating raw animal foods during storage, preparation, holding, and display. PIC is relocating raw beef, raw chicken, and raw porkchops in reach in freezer near dishwasher to prevent cross contamination.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed a male employee prepping food items in kitchen area without a proper beard restraint. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. PIC will purchase the male employee a proper beard restraint.
17A - toilet facilities: properly constructed, supplied, cleaned

Observed women's restroom trash receptacle without a covered lid that is needed for sanitary napkins. CA: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. PIC will purchase a lid for women's receptacle in women's restroom.
