Observed water leaks in the kitchen dishwashing area. The garbage disposal has two bad leaks. Observed a lot of food debris all over the garbage disposal from the leaks. Observed a foul odor coming from the dishwashing/sink/disposal area. Contact EH with a plan of corrective action within 5 days.

Observed rusted metal shelves in the kitchen area. Shelves under prep tables are rusting. Observed soiled rolling carts. Plan of corrective action needed within 5 days. Contact EH with the plan of corrective action.

Observed damaged wall at hand washing sink area located at back door. Observed the hand sink separating from the wall. Contact EH with a plan of corrective action within 5 days.
