Observed the nozzles of the soda dispenser machine unclean on alcohol swab for the lemonade and the starry drinks. CA: 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf. COS: PIC washed, rinsed, and sanitized the nozzles.

Observed ceiling missing. Observed mop area not properly cleaned. Informed PIC to replace missing tile and to clean mop area.

Observed front counter employee remove items from floor and process to put on clean gloves before washing hands. Inform Manager and demonstrated proper hand washing technique to employee.

Handwashing sink not accessible for employee use at all times. Observed handwashing sink blocked with industrial fan. The fan was covered with dust at the time of inspection. (COS) Advised to clean outside after hours.
Observed pretzel in reach in cooler not covered. Informed person in charge that all food item should be covered to prevent cross contamination.
