Observed hot water above 120F in rooms 102 (123F), 201(128F), 205 (130F), 213 (130F). Hot water in all guest rooms shall not exceed 120F
6 - premises free of pests and rodents; pesticide applied in accordance with laws, rules, and label
Regulation: 511-6-2-.13(1) - insect and rodent control
Observed multiple live German roaches in room 205. CA. Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

Observed broken furniture and microwaves unclean to sight and touch in room 213 and 205. Microwaves and refrigerators in 205, 201, 102 were rusting and dirty, curtains were also unclean to sight and touch in all rooms inspected. Also observed A/C units and filters unclean to sight and touch in all rooms inspected. Ca: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed linen and pillows in rooms 102, 201 and 205 with holes stains and debris. Observed holes in comforter in all rooms inspected. C.A. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin
34 - grounds, walkways, porches and hallways maintained; drives provide unobstructed width and proper spacing; graded to drain; trailer spaces marked clearly, areas clean and maintained in good repair

Observed trash cans along hallways without lids. All trash cans should be adequately lined and covered
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed bathtubs not clean to sight and touch in all room inspected. CA. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed unclean and inoperable bathroom vents in all rooms inspected. CA: Toilet, lavatory and bathing facilities shall be mechanically ventilated, and the current ventilation must prevent odor, mold, mildew, and moisture
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means
Regulation: 511-6-2-.13(3) - insect and rodent control-openings
Observed exit doors with gaps at rooms 102, 201 and 205. PIC advised to repair doors and repair/replace weather proofing strips. Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

Observed walls and baseboards unclean to sight and touch with heavy dust debris and stain in all rooms inspected. C.A. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean
