Observed hand sinks without paper towels at: -front hand sink -sole restroom hand sink
COS paper towels were placed at sinks.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed employee wash and rinse a pan without properly sanitizing. COS employee placed pan quaternary ammonium sanitizer for 60 seconds (manufacture's specifications)
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed food handling employee with facial hair longer than 1/2 inch not wearing an effective hair restraint.
Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

Observed leaking plumbing fixtures at 3 compartment sink area.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed the exterior of pizza dough machine unclean to sight and touch. Equipment needs to be cleaned to prevent contamination of food products.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(r),(s) - frequency, removal of refuse, recyclables and returnables often enough to minimize odors and attracting pests; receptacles or vehicles, removal from premises in approved manner (c)
Observed excess trash stored outside trash receptacle behind facility. Facility must have dumpster area cleaned to minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents.
