PIC placed wet wiping cloth inside sanitizer bucket with sanitizer concentration of 100ppm chlorine.

Observed trash receptacles in the women's restrooms without covered lids.

Toilet Room Receptacle, Covered (C) (h) Toilet Room Receptacle, Covered. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

Discussed with PIC, that the women's restroom needs to be equipped with covered trash receptacles.

Observed multiple TCS foods including raw chicken, refried beans, and cooked rice that were above the required cold holding temperature of 41 or below.

Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. PIC voluntarily discarded the out of temperature raw chicken, refried beans, and cooked rice.

Observed black, organic frozen debris inside the ice bin, which was dripping onto the ice below.

Discussed with PIC that all ice in the ice bin must be burned/discarded, and the ice bin must be thoroughly cleaned and sanitized by the end of the day, before it can be used again.
