A large hole (about 2inches wide) as well as gap at the bottom of the back exit door was observed. CA: Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Filling or closing holes and other gaps along floors, walls, and ceilings; Solid, self-closing, tight-fitting doors.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed tiles missing around the grease trap in the kitchen and one cracked tile in the kitchen. CA: All physical facilities shall be maintained in good repair.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(i) - light bulbs, protective shielding (c)
Observed no light bulb shielding on the light bubs located in the kitchen above the prep table and chest freezer. CA: light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food; clean equipment, utensils, and linens; or unwrapped single-service and single-use articles.
