PIC stated that they will send me an email of the signed employee health reporting agreements. Discussed with PIC that they must have the signed agreements on site at all times.

Observed back door in main kitchen with gap on the right side of the door. Observed back door near restrooms with air gap on the right side and the bottom of the door. CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by Solid, self-closing, tight-fitting doors. Discussed with PIC that they can get weather strips to cover the openings on both back doors. This will prevent pests from entering the establishment.

When the three compartment sink is emptied, the water rises above the rim of the air gap pipe and overflows on the floor. Discussed with PIC that they have 10 days to repair plumbing.
