Observed a plastic container sitting in the hand washing sink upon arrival. CA: A handwashing sink shall be maintained so that it is accessible at all times for employee use. A handwashing facility may not be used for purposes other than handwashing. COS: Food employee removed and discarded plastic container.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Observed a box of to-go food containers stored on the floor in the dry storage area. CA: Food shall be protected from contamination by storing the food in a clean, dry location; where it is not exposed to splash, dust, or other contamination; and at least 6 inches (15 cm) above the floor. COS: Food employee moved the box of to-go food containers to a shelf in the dry storage area.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed two flying insects in the facility during the inspection. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; using methods, if pests are found, such as trapping devices or other means of pest control, and eliminating harborage conditions.

Observed the entrance/exit door held open by a lobby chair. CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by filling or closing holes and other gaps along floors, walls, and ceilings; closed, tight-fitting windows; and solid, self-closing, tight-fitting doors. COS: Food employee closed the entrance/exit door and returned chair to lobby area.
13A - posted: permit/inspection/choking poster/handwashing

Observed an expired food permit posted in the facility for public view. CA: Post a valid permit in a location in the food service establishment that is conspicuous to consumers.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed a pool of water at the bottom of the walk-in cooler. Observed food debris and spillage on the floors of the walk-in freezer and reach-in cooler. Advised to clean. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed soiled and missing ceiling tiles in the main kitchen. CA: All physical facilities shall be maintained in good repair.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(h) - dressing areas and lockers, designated and provided (c)
Observed employee clothing, bags, and personal belongings stored on shelves with dishes and cans. CA: Lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions.
