Regulation: 511-6-1.04(1) - safe, unadulterated and honestly presented (p)
Condition. Food shall be safe, unadulterated, and honestly presented.Observed four (4) bottles of assorted alcohol with multiple fruit flies in direct contact with alcohol for customer consumption stored on liquor display shelves in bar area. Corrected on Site: Food workers voluntarily discarded said food items.
1-2B - certified food protection manager
Regulation: 511-6-1.03(3)(a) - food safety manager certification (pf)
Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation as specified within paragraph (d) of this subsection. At the time of inspection facility lacking food safety manager. Owner of establishment is required to employ a Certified Food Safety Manager within 30 days of this inspection. 6/9/26
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
outer openings of a food service establishment shall be protected against the entry of insects and rodents by: (i) Filling or closing holes and other gaps along floors, walls, and ceilings; (ii) Closed, tight-fitting windows; and (iii) Solid, self-closing, tight-fitting doors. Observed rear door to establishment not protected from the weather and the entry of insects and rodents. Permit holder is advised to close gap at bottom of rear exit door by 5/11/26 and email pictures to inspector via email.
