4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed several pizza screens used to hold pizza as it go through pizza oven unclean at the time of inspection. Observed interior of ice machine with specs of debris. Discussed with PIC that all food and non food contact surfaces shall be cleaned regularly and by sight and touch to prevent buildup. COS- PIC moved pizza screens to 3 compartment sink to be washed, rinsed and sanitized. PIC started to melt ice in ice machine and stated ice machine will be washed, rinsed and sanitized before use.

Observed food contact surface of the lids stored directly on unclean container. CA:Equipment, Food-Contact Surfaces, and Utensils.
- Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS- PIC removed container and voluntarily discarded lids directly touching container.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed containers holding dishes stored as clean with an accumulation of food debris. Observed pizza stand unclean. Observed interior of coolers unclean with accumulation of food residue. Observed cooler gasket unclean with accumulation of food residue. Observed shelves in back area with accumulation of flour. Observed shelf with heavy accumulation of dust. CA:2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
- Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC has 72 hours to deep clean kitchen and all surfaces.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of stains and dust on walls and ceiling in main kitchen. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
