Observed no signed Employee Health Reporting Agreements available during the time of inspection.

Employee Health Reporting Agreement forms will be provided to the facility. All food employees shall review, complete, and sign the forms, and the completed forms shall be maintained on-site and available for review upon request.
2-2E - response procedures for vomiting & diarrheal events
No vomit/fecal kit or EPA registered chemical was observed during time of inspection.
CA: Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.Pf
Advised to obtain a vomit/fecal cleanup kit or an EPA-registered disinfectant within 10 calendar days.

Observed the area beneath the kitchen sink with an accumulation of debris and residue and not clean to sight and touch during the time of inspection.
CA:(d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
PIC is advised to routinely clean and maintain all nonfood-contact surfaces, including areas beneath and around equipment, to prevent the buildup of dirt, grease, and other contaminants.
