Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. Observed approximately 5 pounds of sliced tomatoes,Five, 5 pound packets of sliced american cheese tested, 12 quarts of Light heavy cream and 3 packets of chopped iceberg lettuce tested between a range of 44-45 F in walk in cooler at rear of establishment. As per PIC said food items were placed in walk in cooker one day prior inspection. Ambient temperature of equipment was 43F. COS: All food items were discarded by PIC. Corrected On-Site. Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above. Observed, Breaded chicken patties and fish fillets tested at 107 and 111 respectively in warmer in front food prep area. COS: All food items were reheated to at least 165F by PIC. Corrected On-Site.
2-2D - adequate handwashing facilities supplied & accessible

Hand Drying Provision. Each handwashing sink or group of adjacent handwashing sinks shall be provided with a continuous paper towel system that supplies the user with a clean paper towel. Hand wash sink in male restroom observed without paper towel. COS: PIC replenished paper towel. Corrected On-Site.
12B - personal cleanliness

Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Observed food worker wearing wrist watch while preparing hamburgers and chicken sandwiches for customers order. COS: Food worker removed wrist watch. Corrected On-Site.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. Observed food worker not wearing a hair restraint while preparing hamburgers and chicken sandwiches for customer's order. COS: Food worker obtained a hair restraint. Corrected On-Site.
18 - insects, rodents, and animals not present

Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Solid, self-closing, tight-fitting doors. Observed outer opening between rear exit door and door frame in the rear food storage and non food storage area.

Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Observed an accumulation of grease and dust particles on exterior of deep fryers in main kitchen.
