Observed time/temperature control for safety (TCS) food cold holding above 41F.See temperature log. CA: Informed PIC that TCS food must be held at 41F or lower for cold holding. COS: Food cold holding for more than 4 hours was voluntarily discarded by person in charge and food improperly cold holding was placed in the walk in freezer.

Certified food safety manager was not posted. CA: Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) to ensure food safety is being managed within the food service establishment during all hours of operation. COS: CFSM was posted.

Observed a plastic container in the hand sink next to the dish washer. Observed employee fetching water at the bar hand sink. CA: A handwashing facility may not be used for purposes other than handwashing. COS: Container was removed from the hand sink.

Observed two hand sink in the main kitchen with no paper towel/ and paper towel dispenser not properly dispensing paper towel. Observed no paper towel at the bar hand sink. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels; A continuous towel system that supplies the user with a clean towel. COS: Paper towel was provided to each hand sink.

Observed a unclean knife with heavily food accumulation/debris stored as clean. CA: surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: At any time when contamination may have occurred. CA: Knife was removed to be washed, rinse and sanitize.

Observed the inside of the ice machine to be unclean with black-like substances. Observed a piece of hair at the surface of the ice machine gaskets. COS: Hair was removed. Inform person in charge to unplug ice machine to proceeds with cleaning process.

Observed food employee with long dreads/hair prepping food without a proper hair restrain. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: Hair net was worn.
