Owner could not provide a documents for gas-fired equipment inspections. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions

Observed the main door entrance with opening(gaps/day light). CA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed stains on linens/bed coverings in the following rooms: Rooms 4 and 6. Observed stains on the single coach in room 2. CA: Informed person in charge that beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. Operator unable to produce pest control records at the time of inspection
