Observed sticky trap in main kitchen with approximately 8 dead roaches. Dead insects, rodents, and other pests shall be removed from the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. COS trap was removed from kitchen.

Observed build up of grease on hood and black organic growth on outer surfaces of sauce pans. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Observed accumulated grease on floor and wall behind cooking equipment (fryers). Also observed vents in main kitchen with accumulation of dust. PIC advised to clean areas. CA. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products
