Observed TCS foods (meat loaf, mac & cheese)in reach in cooler without date markings. PIC reported items were made two days prior to inspection. COS PIC discarded items.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed food handler preparing foods wearing bracelets on wrists. COS food handler removed bracelets. CA. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed food handler preparing food without adequate hair restraint. COS. Food handler donned hair net. CA. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
18 - insects, rodents, and animals not present

Observed two dead roaches in front service area beneath counter. Also observed approximately 12 dead roaches in main kitchen along walls behind equipment. PIC advised to remove all dead roaches and use counter measures such as treatment and sealing all openings to rid facility of roaches. CA.The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Observed no food permit posted. C.A. Post the permit in a location in the food service establishment that is conspicuous to consumers. Facility also has unpaid permit fee. Correct by 8.21.25

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed missing ceiling tiles in storage room and inside main kitchen area. Advised to replace missing ceiling tiles.
