2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed handwashing sink with residue from smoothie mix on surface of handwashing sink. Informed PIC that sink is for handwashing only. PIC clean surface of handwashing sink and inform staff to use for handwashing only. COS CA: 2. A handwashing facility may not be used for purposes other than handwashing. Pf
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed bananas , blueberries, strawberries and pineapple stored in prep top cooler uncovered and exposed to cross contamination of flies and fruit flies. Observed frozen yoghurt and butter pecan ice cream without lid in reach in freezer that is in a state of disrepair. Informed PIC to cover all food item to prevent cross contamination from splashing water and flies. CA: Food shall be stored in packages, covered containers, or wrappings to be protected from overhead contamination. COS: PIC provided lids for the food that were uncovered.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed (2) employees without proper hair restraints. Observed another employee with flowing hair. Informed PIC (no hair restraint available for employee). CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: Employees put hair in ponytail.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(f) - fingernails (pf)
Observed one employee with false and painted nails with protruding decorations on them reach into freezer to grab a packaged bowl where frozen yoghurt and butter pecan ice cream were stored uncovered. CA: Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers. Pf Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food. Pf COS: Employee donned gloves after PIC and employee were informed that gloves must be worn for employees with long, false, and/or painted nails.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed the old previous inspection posted with "100 A" score during the time of inspection. The most current previous inspection score was an "85 B". CA: 1. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. 2. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information. COS: PIC replaced score with the score provided during this inspection.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed reach in freezer in front food service missing one of two sliding doors, leaving the freezer and the food inside open to overhead contamination. Observed one door reach in cooler in main kitchen with food inside in disrepair. PIC stated that the food inside of the is not being used and will be discarded. Ca: 1. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. 2. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. The one door reach in cooler was labeled "do not use" and the food inside of the cooler was discarded (two containers of plain Greek yoghurt.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floors to the facility in disrepair. A loose piece of the floor in the front food service area, in front of the prep top cooler stuck to the bottom of inspector's shoe and lifted up almost causing inspector to trip. The floor in the back area are missing tiles. CA: All physical facilities shall be maintained in good repair.
