Observed accumulation of debris in ice machine in and around dispenser opening. All food contact surfaces are to be cleaned and sanitized frequently enough to prevent such accumulations. Person in charge will empty, clean and sanitized ice bin at end of day.

Observed live insects in dining area and in rear of food prep area near ice maker and entrance to walk in cooler & freezer. Facility is to be maintained pest free through the use of licensed pest control applicators on a routine basis. Facility will receive extra treatment as soon as possible. Facility will provide proof of treatment to health department.

Rear kitchen area has accumulations of debris, other organics, and spilled liquid in various spots beneath storage shelves. Facility must be maintained clean to prevent the attraction and harborage of insects and other pests. Plan discussed with Manager to have area cleaned at end of shift. Increase routine cleaning schedule as well.

Observed live flies throughout entire kitchen and dining area. Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

1. Routinely inspecting incoming shipments of food and supplies;

2. Routinely inspecting the premises for evidence of pests;

3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

4. Eliminating harborage conditions. Pest control last serviced on May 14, 2024 by Beyond Pest Control Justin Blackovic #SP37053. PIC Stated she will call pest control to service facility for flies ASAP. Priority foundation item must be corrected within 10 calendar days. Person in charge will email pest control records to me.

Observed previous inspection not posted in public view. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. COS: PIC posted inspection in the front in public view.
