4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed raw beef stored on shelf with raw chicken in the RIC. CA: 1. Food shall be protected from cross contamination by: (ii) Except when combined as ingredients, separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by (II) Arranging each type of food in equipment so that cross contamination of one type with another is prevented. PIC rearranged the pork and chicken based on cooking temperature.
1-2B - certified food protection manager
Regulation: 511-6-1.03(3)(a) - food safety manager certification (pf)
Observed facility without current Certified Food Safety Manager certification. CA: Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation as specified within paragraph (d) of this subsection. PIC will obtain within 30 days.
4-2A - food stored covered

Observed containers of raw diced chicken, sliced beef, shrimp, broccoli, celery and chopped carrots in the reach in cooler not stored covered to prevent contamination. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC placed lids on the containers.
18 - insects, rodents, and animals not present

Observed facility with front door open. CA: Except as specified in paragraphs 2 and 5 of this subsection, if the windows or doors of a food service establishment, or of a larger structure within which a food service establishment is located, are kept open for ventilation or other purposes or a temporary food service establishment is not provided with windows and doors as specified under paragraph 1 of this subsection, the openings shall be protected against the entry of insects and rodents by: 16 mesh to 1 inch (16 mesh to 25.4 mm) screens; Properly designed and installed air curtains to control flying insects; or Other effective means. PIC closed door.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed fumehood in need of cleaning. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC will have fumehood cleaned.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floor drain in kitchen area holding water. CA: All physical facilities shall be maintained in good repair. PIC will have floor drain repaired.
