Observed raw eggs and turkey stored above ready to eat shred cheese, cut avocado, cut red pepper, shred carrots, cherry tomatoes, spring mix, raspberries and blue berries and cream cheese. CA: Raw food must be stored below ready to eat foods based on cooking temperatures.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed facility washing and rinsing dishes at 3 compartment sink without sanitizing in the 3rd compartment sink. CA: Food contact surfaces and utensils shall be washed rinsed and sanitized. COS:PIC set up sanitizer ewater to sanitize dishes in 3rd compartment of sink at 50 ppm chlorine.
18 - insects, rodents, and animals not present

Observed several small flying insects on prep sink in front service area. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises. Pest control required to treat for pests by June 7, 2026 and documentation of treatment must be maintained on site
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(d) - backflow prevention, air gap (p)
Observed no air gap installed for interconnected prep sinks, hand sink, 3 compartment sink and mop sink. Must be corrected by a licensed plumber by June 7, 2026
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed hand sink leaking water onto floor and into a bucket in front service area. CA: Physical facilities must be maintained in good repair.
