Observed cold holding time temperature control for safety food cold held at greater than 41 degrees Fahrenheit CA: Food must be cold held at 41 degrees F or below. PIC discarded some foods. Must be corrected within 3 days.

Observed potentially hazardous food not held at 135 degrees Fahrenheit or above.(foods asterisked below) CA: Time/Temperature control for safety foods must be hot held at 135 Degrees F or above. PIC moved food to cooler
2-2A - management knowledge, responsibilities, reporting

The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. Observed walk in cooler door and flooring in smoker area not constructed of corrosion resistant, nonabsorbent, and smooth materials. CA: Nonfood contact surfaces must be smooth durable and easily cleanable.
15C - nonfood-contact surfaces clean

Observed an accumulation of excess black residue and standing water on floors in smoker area. CA: Nonfood contact surfaced must be maintained clean.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed walk in cooler fan condenser leaking a steady stream of water onto floor in walk in cooler. CA: Physical facilities must be maintained in good repair.
