Observed cold TCS food (shredded cheese, cottage cheese, potato salad, dairy and egg based sauces) stored above 41 F at the salad bar. COS. Items discarded by the PIC. Code: TCS food that uses cold holding as a control for safety shall be held at 41 F or below.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed accumulation of mold/mildew-like substance on the inside of drink station nozzles. RCA: All drink station nozzles shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation. CA: Food contact surfaces shall be clean to sight and touch.

Observed facility did not have the allergen disclosure posted in the facility. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.

Observed containers of food and sauces that were not labeled. COS. Labels placed. Code: Working containers holding food or food ingredients that are removed from their original package shall be labeled with the common name.

Regulation: 511-6-1.05(4)(b) - fixed equipment, spacing or sealing (c)
Observed hand sink in main kitchen near the pizza oven was not properly attached to the wall. CA: Equipment that is exposed to splash or spillage shall be properly sealed to the walls.

Observed gasket not properly attached door of reach in freezer. CA: Equipment shall be maintained in a state of good repair.

Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
