6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed cold TCS food stored above 41 F (tomatoes, cheese, lettuce, sour cream, whipped cream). COS. Discarded. CA: TCS food that uses cold holding as a control for safety shall be held at 41 F or below.
6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed hot TCS food stored below 135 F (chicken). COS. Discarded. CA: TCS food that uses hot holding as a control for safety shall be held at 135 F or higher.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed food stored uncovered. COS. Covered with lids and plastic wrap. CA: Food shall be stored covered.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed that wet wiping cloths were stored on counters. COS. Placed into sanitizer solution. CA: Wet wiping cloths shall be stored in sanitizer solution between uses at the appropriate concentration.
12D - washing fruits and vegetables
Regulation: 511-6-1.04(4)(g) - washing fruits & vegetables (pf)
Observed that there was raw shrimp in the vegetable sink. COS. Removed and sink was washed, rinsed, and sanitized. CA: Produce shall be washed in a sink that is designated and used for that purpose only.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(m) - outer openings protected (c)
Observed that the rear door by the public restrooms was not self closing and was kept/propped open. CA: Outer openings shall be self-closing and tight-fitting.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed several areas of the floor was the concrete had been broken and removed, the floor is not longer smooth and easily and is not maintained in a state of good repair; observed holes in the walls; observed accumulation of debris under equipment and shelves. CA: Facility shall be maintained in a state of good repair; facility shall be thoroughly cleaned and cleaning freuqency shall be enough to prevent accumulation of debris.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee belongings stored with and above food and food contact surfaces. COS. Moved. CA: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
