Observed food stored uncovered in reach in freezer and walk in cooler. COS. Lids placed. CA: Food shall be stored covered when not actively in use.

Regulation: 511-6-1.04(7)(d) - other forms of information (c)
Observed that the facility did not have written disclosure of the major food allergens used in the facility. CA: The facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.

Observed leaks from faucets of multiple sinks. CA: Pluming systems and fixtures shall be maintained in good repair.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
Observed containers stacked while wet. COS. Unstacked to properly air dry. CA: Equipment and utensils shall be properly air dried prior to stacking.

Observed accumulation of debris on and in equipment throughout the kitchen. CA: Equipment and non-food contact surfaces shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.
