Observed that the facility is keeping chemicals (car oil, DW40, cleaning sprays) with and above food and food contact surfaces on the shelf by the backdoor. COS. Items moved. Code: Chemicals shall be stored in a manner that prevents contamination of food and food contact surfaces.

Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed that all of the containers of various spices and seasonings, along with squirt bottles, on the line were not labeled with the common name of the product. Code: Non-easily identifiable foods shall be labeled with the common name of the product.

Observed that the facility does not have written notification of the major food allergens used in the facility. Code: The facility shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed that the facility is using bowls as scoops for raw chicken and beef and is storing the bowls in the product. COS. Bowls removed and scoop placed. Code: Scoops shall have handles and the handles shall be stored above the level of the product, if stored in the product.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris, grease, and food on floors, particularly under equipment and shelves in the water alley and near the cook line; observed accumulation of grease on ceiling tiles. Code: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation of debris.
17D - adequate ventilation and lighting; designated areas used

Observed employee items (jackets) stored with and above food and food contact surfaces on the shelf near the backdoor. COS. Items moved. Code: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
