2-1B - hands clean and properly washed
Regulation: 511-6-1.03(5)(c) - when to wash (p)
Observed employees handling trash/debris who then changed gloves before working with food but did not wash their hands, observed employees enter the kitchen and begin working with food and making drinks without washing their hands, observed employees put their hands in their pockets and begin working with food without washing their hands. COS. All employees were educated on when to wash and washed their hands. Code: Prior to working with food, when changing tasks, and when contamination occurs, hands shall be washed.

Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed that shaker bottles and squirt bottles with spices and suaces on the lines were not labeled with the common name of the items inside. COS. Labels added. Code: Non-easily identifiable foods shall be labeled with the common name of the product.

Regulation: 511-6-1.04(7)(b) - honestly presented (c)
Observed that the facility is selling fish and labeling it as, "white fish". Code: Food shall be offered in a manner that does not mislead or misinform the consumer.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(a) - equipment & utensils, air-drying required (c)
Observed containers were stacked while wet. Code: Containers shall be thoroughly air dried prior to stacking.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris on the floor in the main kitchen under shelves and equipment, particularly on the line, and accumulation of grease and debris on walls and ceilings in the main kitchen; observed water damage and missing ceiling tiles in the bar storage area; observed heavy accumulation of debris on the floor under the sinks at the bar. Code: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee belongings stored with and above food and food contact surfaces in the main kitchen and the back prep area. COS. Removed. Code: Employee belongings and activities shall be in desginated locations that prevent contamination.
