Observed live roaches in 2 rooms inspected. Roach crawling on bathroom sink in room 206. Roach crawling on door in room 204. Corrective Action: Owner was notified. Monthly Pest Control Contract with Coosa Valley Exterminating was verified. Exterminator will be contacted for immediate treatment of all affected rooms and surrounding rooms. Treatment frequency must be increased to every 2 weeks. Written documentation is required. These rooms should not be rented until cleared by Exterminator and Health Dept. This will be rechecked

Observed broken or missing smoke detectors in rooms 204 and 211. Corrective Action: Owner was notified. Batteries and/or smoke detector units were replaced in all rooms onsite and proper operation was confirmed

Observed food build up on interior surfaces of microwave in room 206. Corrective Action: Owner was notified. Housekeeping staff was assigned to clean and sanitize microwave onsite. This should be done between each guest

Observed broken or missing smoke detectors in rooms 201, 207, 212, and 216. Corrective Action: Owner was notified. Batteries and/or smoke detector units were replaced in all rooms onsite and proper operation was confirmed

Observed rooms with unapproved cooking equipment. Hot plate with cooking pot found in room 212. Corrective Action: Owner was notified. Resident was instructed to remove all cooking equipment from room onsite. PIC and residents were reminded that no unapproved cooking equipment should be used in rooms due to potential fire hazards. Only provided microwaves should be used

Observed heavy accumulation of trash, food debris, and bottles on floors and furniture of room 216. Corrective Action: Owner was notified. Housekeeping was assigned to clean this room immediately. Management was also instructed to have all rooms checked regularly, especially extended stays, to ensure these rooms stay clean and sanitary

Observed smoke detector not working in room 216. Corrective Action: Owner was notified. Battery was replaced in smoke detector and proper operation was confirmed

Observed multiple rooms with unapproved cooking equipment. Air fryer and Cooking pot found in room 201. Crock pot observed in room 212. Corrective Action: Owner was notified. Residents were instructed to remove all cooking equipment from rooms onsite. PIC and residents were reminded that no unapproved cooking equipment should be used in rooms due to potential fire hazards. Only provided microwaves should be used

Observed paint cans, tools and several pieces of broken/unused equipment stored on floors in laundry area. Corrective Action: PIC was notified and is in the process of cleaning up laundry area and removing all broken/unused equipment throughout facility. This will be rechecked

Observed heavy build up in air conditioning filter in room 212 Corrective Action: PIC was notified. AC filters in all rooms will be removed and cleaned. This will be rechecked

Observed damaged ceiling in main laundry facility behind motel. Corrective Action: Owner is aware of this issue and is coordinating repairs. This will be rechecked

Observed smoke detectors not operating or missing in rooms 205, 206, and 230. Corrective Action: Owner was notified. Smoke detectors were replaced in all units and verified to be operating properly
