Observed several potentially hazardous TCS items cold held above 41 degrees in the prep top cooler. Sliced American cheese 50 degrees F, Sliced provolone cheese 48 degrees F, sliced tomatoes 44 degrees F. Prep top cooler has had issues before and issues persist after being serviced on numerous occasions. CA: PIC removed TCS items to be cooled to 41 degrees.

Observed black build-up on the interior surfaces of ice machine in main kitchen.
Corrective Action: PIC was notified and educated on proper cleaning procedure and frequency. PIC will designate an employee to drain and properly clean ice machine and contact the Health Department to recheck before use.

Observed broken tiles in walk-in cooler along transition to walk-in freezer.
Corrective Action: Discussed with PIC. The floor in the walk-in cooler must be smooth and easily cleanable. PIC says they already submitted a work order. This will be rechecked by the Health Department in 30 days.

Observed multiple potentially hazardous TCS foods cold-holding above 41 F in prep-top cooler (sliced tomatoes, cheeses, rib meat, ham, turkey, corned beef.) Per PIC, there are known issues with this prep top cooler not holding temperature and work orders have been submitted. The bottom portion of the unit is currently out of use due to a known leak.
CA: All foods in prep top were discarded as it was unclear which items were from the night before or at what point they were restocked during the morning shift. Due to limited cooler space in the facility, the decision was made to use time as a public health control for TCS items for a limited time (48 hours) until the unit is properly serviced by a licensed technician and documentation of service is provided. The cooler will not be used for cold holding until rechecked by the Health Department.

Observed black build-up on soda dispensing nozzle in drive thru window. CA: Discussed cleaning method and frequency with PIC. PIC removed and properly cleaned and sanitized soda nozzles in drive thru and dining room. PIC will educate employees on cleaning frequency and methods, including the use of a brush, to ensure all drink nozzles are properly cleaned.

Observed heavy buildup of ice from frozen condensation on boxed food items in walk-in freezer. CA: Discussed with PIC. Boxed food items were found to be sealed in plastic and were not directly contaminated. All boxed food items were relocated away from condensation leak. Licensed professional will service freezer and correct issue. This will be rechecked.

Observed ice build-up on transition from walk-in cooler to walk-in freezer preventing the door from closing properly. Ice build-up also noted around door of walk-in freezer. During inspection, walk-in freezer door was found propped open several inches after catching on the transition with ice. CA: Discussed with PIC. PIC will have the walk-in cooler and freezer serviced. This will be rechecked by the health department.
